Health Physicals and Birth Certificates

HEALTH PHYSICALS AND BIRTH CERTIFICATES State law requires evidence of a physical examination completed within six months prior to the entrance into seventh grade. Physical examinations required prior to participation in athletics will satisfy this requirement. Physical examinations are required of all students prior to athletic participation. Immunization records must also be current. Official birth certificates are required of students when they enroll in the Boone Central School District for the first time.
