New Student Registration

Home Calendar

New student enrollment is done at the Columbus Public Schools Administration Building located at 2508 27th Street.

Students must provide the following for enrollment:

  • Birth Certificate
  • Immunization Record
  • Court papers when circumstances warrant.

The following must also be completed:
  • Personal Enrollment Form
  • Release of Information
  • Language Survey
  • Birth Certificate Form
